Do all broadcasts output via the optical out on the Sky+ box, or only those that are in DD? In other words, if I'm connecting the Sky+ box to my AV amp, will I be able to get audio just via the optical, or will I still need to connect the phonos for some channels/programmes?

You will be able to hear all channels fine using this connection.
As you are probably aware only a couple of movie channels broadcast in DD. However you will still be able to listen to all channels that are broadcast in dolby surround etc.
    :)
