Picture Goes To Small Format

Picture on my LG OLED goes to a reduced size format by itself. I've attached a picture of what is happening. I'm assuming this is a result of the signal from the cable box? Any reason why it could be something wrong with the TV?

How is your cable box connected to your TV...RF or HDMI?
What aspect ratio is your cable box set to?

I'm having the exact same issue on my 70" 6570. This just started last week. I'm connected to my sat box by HDMI. Any help would be appreciated.

As a side note, this only seems to happen when a commercial ends and the program starts again.
