Plug the DVD into the TV using a HDMI cable, then an optical cable from DVD to Amp. Plug Sky HD into TV using HDMI lead, then optical from Sky to Amp. Change settings in Sky services menu to allow dolby audio output.

You could put the HDMI's from the Sky and DVD to the amp then have 1 from amp to TV, but the only difference would be slightly simpler switching so I wouldn't bother.

Obviously be careful with the speakers making sure the wires are correct at both ends (positive to amp and speaker). Also make sure the back speakers are connected to the surround speaker posts on the amp and not the surround backs.

i think you will need a co-axial cable for the sub woofer that goes to a specific connection on the back of the amp - someone else can confirm this.
