I've never seen this mentioned. Every now and then in darker scenes there will be the odd white pixel. If you have Lost on Blu-ray, every single time the "Lost" opening logo starts, no matter which episode in season 1, you'll see one at the very top of the screen near the left side towards the end of the sequence. In exactly the same place, everytime.
Now, if this was cables, it would be random, non reliably repeatable. But it gets better: I've seen it in cinema watching Uncharted. It is not cables.
It seems it's baked into the source material, maybe a result of the development process? It's like a single pixel of white, similar to grain, every now and then. You can catch it, rewind and it'll show again.
Not sure if anyone else has experienced this or can explain it. In the episode "Do No Harm", Jack is playing a piano and there's a lot of this type of noise there.
