I have had this on a customer's PC once and it turned out to be caused by a weak/dodgy power supply.

If you haven't checked the temperature of the video card, that is also something worth looking at.

I have a 750w OCZ, should be enough right? And yeah I've checked, I'm
Hitting 45 Ish during gaming, and in the 30's/20's at idle

OCZ psus are in general rock solid, been using their budget StealthXStream 1 & 2 series for a long time in my/customer PCs.

Consider what is easier for you as a next step, swap the PSU with a friend for a week or re-install Windows?

I used to get that with a gtx580, win7 64 bit, but I think that one of the recent nvidia driver updates has fixed it for me (within the last 1-2 months). 290.53 beta is available now. As it's beta the nvidia control panel doesn't tell you about it automatically though I think.
