Not a private plate, but my GF had to fill out one of those forms to get an NI 206 registered with the DVLA (already on there with the DVLNI or whoever they are!).
You can get all the details off the V5 document for size, weight etc. They're not too picky about it - was relatively harmless. Reason it needed sorting, was because she needed an MOT, but wasn't prepared to take it back to NI.
It wasn't recognised by MOT stations (the NI MOT system seems to be more strict), so she was advised to get it registered. Standard, it seems, but a bit of a pain when you aren't expecting it.
