When I did my 3.21.62 update, I put file on USB stick (same as you), and plugged into TV.Then turned TV on and it recognised firmware upgrade.
I'm sure the checking for new firmware via the TV menu (Menu, Red button for Customer Support etc) doesn't work anymore as I went back to old version and then tried that and it didn't find the 3.21.62.

Did you try to upgrade from USB with the aerial unplugged? Apparently you have to do that or it will come up with a fail message.
It may be the file is not compatible, not sure.

Update:
Just tried it with and without aerial in, and it doesn't get recognised.Even tried to load the 3.3.18 file but no good.
Must be incompatible, which makes sense as the file is on the LG site for the 450 but not 490.
Could be something to do with the Freeview HD side of it as that's the only difference between the 450 and 490.

Hello

I've been working away so not posted for a while.
Sorry Nick, but I gave up on Nero I uninstalled it because I just couldn't get it to stream live feeds (and I got a PS3)
Thought i would post as being curious about the newish firmware I too put it on USB and installed.
First of all, I was pleased it didn't mess up my custom settings (other than initially a minor issue with the blue power indicator being on when it was set to off.
I was hoping Netcast may get a upgrade but no- Smart TV has won and we're out to dry so to speak.
Iplayer streams fine with only the initial buffer on both HD and standard def.
I'm getting a 7meg connection normally.
certainly smoother operation with the remote, Netcast front page loads much faster but Iplayer itself still gets the little flying plane for a bit.
Sound wise I've tweaked to standard via iplayer- it distorts like mad if you apply infinite sound.
For all other inputs I've set it to Music, bass and treble both at 40 and infinite sound on.
Motion seems more refined- but it's too early to really tell.
Inbuilt Freeview BBC HD occasionally goes 1080p (as does 4hd) which is always nice and they look spectacular with my picture settings which I haven't changed.
The PS3 handles BLU RAY with ease- as it should.
My former LG BLU-RAY LD390 Was consigned to digital heaven as the motion with some discs was plain awful (LEIGION for example).
I get Youview next week and have ditched Sky hd (pic quality on that pre update was poor!- but that's sky for you).
I will keep you informed of my relationship with the new firmware.
