Call Forwarding Active message for every call I make. Why?

Any time I make a call with my Iphone 3GS, a message box pop up saying "Call Forwarding Active" with a button labelled "Dismiss".

I've check call forwarding in my phone settings and it's off.

I'm with T-Mobile. Can anyone help?

Thanks

There's a fix if you have an unlocked phone with the cydia application.

Details if you want them.

Launch Cydia

Open 'More Package Sources'

Load 'iPhone-Notes.de'

When thats done, open 'sections' menu, scroll down to 'tweaks'

press 'ForwardMsgFix'

That did it for me.

I have another solution:
Open the phone pad:
Type: ##002# -> Hit call button -> Then hit Dismiss

Try the call again, you won't see the message box.
