Home Cinema keystone wallplates

I am getting closer to adding equipment to my home theater in our new construction house. I have all the wires ran and they have now foamed the walls and sheetrocked. I noticed today that i messed up I ran my phone, cat5, 2 hdmi and a set of rca into the same double box. I ment to put the 2 hdmi and the rca in one box and the phone/cat5 in another. The reason for the seperation was I left about 3 feet of cable for both the hdmi and the rca and was going to use a double gang pull threw and then a standard keystone plate for the phone/cat5. Now my question after all the dribble is do they make a combo double gang keystone / pullthrew cover. I need these 2 combined into a dobule gang:

I would just stuff the cable back into the walls and use the keystone but when you use foam insoluation its not a easy task to stuff wire back up Trust me i tried lol.

here is a crude ruff drawing but you get the idea, a pull threw and a keystone on a double gang cover. Maybe keystone not the best word. Quickport inscert and pullthru maybe?
