RF extenders

I've noticed a couple of remotes have RF capabilities with a "remote extender". Is this a separate peice of equipment?
Do you need a separate transmitter in the room you are pressing the remote control in and a separate receiver ?
What I'm looking for a a remote that I can use anywhere in the house to change channel over on my sky digibox for example. I already have a remote expender thingy which has a transmitter and a receiver but this would require a transmitter in every room
