So the AV amp and the PS3, Sky HD+, Wii etc are all connected in a room a floor below. The TV and the speaker system are on the floor above.

We need at least a 50m CAT5/6 solution to go between the amp and the TV. Now the TV has no speakers and depending on if there is a lag with the wireless controllers - the consoles may be brought up and connected directly into the TV. But since the TV has no speakers - a digital optical cable running from the TV needs to go back down the same distance to the amp. So we need HDMI going in one direction with optical going back in the other.

Any suggestions for doing this with the least amount of boxes etc?

I would also check that your TV optical output will actually send out sound input from the Wii and PS3 as it is not uncommon to find that the TV will not send out over a digital link sound that it receives over analogue (Wii for example)

Are you expecting to deliver 5.1 to the AV Amp over the 'extended' connection?
